The St.Mary Cathedral and St. Ann Church Pastorate requires a godparent or sponsor to complete a form, which is an acknowledgement that an individual is in good standing with the Church and is eligible to serve in this role.  A pastor's signature or parish representative along with the parish seal is required. Please fill the form out completely before submitting the document to our office.
